Continuous Synopsis Record (CSR)
Continuous Synopsis Record — the onboard history of a ship's flag, ownership, management, classification and security certification, required by SOLAS regulation XI-1/5 and issued under the ISPS Code.
What this certificate is
The Continuous Synopsis Record is a ship's history, carried on board. SOLAS regulation XI-1/5 requires every flag State to issue one, recording who has flagged, owned, chartered, managed, classed and security-certified the vessel — and to issue a fresh one each time any of those change, so the chain is never broken. A port State officer reads it to see a ship's whole past in one file, which is why gaps in a CSR attract attention that the ship's other certificates do not.
Issued under
This Continuous Synopsis Record is issued under the provisions of the International Code for the Security of Ships and of Port Facilities (ISPS Code) and SOLAS regulation XI-1/5, under the authority of the Government of the Republic of Haiti. It is to be kept on board and made available for inspection at all times, and must remain with the ship for the whole period of her registration under this flag.

Who needs this certificate

What to send the registry
- Current Certificate of Ship Registry and IMO number

- Name and registered address of the ISM Company, its safety-management address and Company identification number
- Name of every classification society with which the ship is classed
- The organisations that issued the Document of Compliance, the Safety Management Certificate and the International Ship Security Certificate
- The complete set of CSR documents issued by the previous flag State, where the ship is reflagging
Practical notes
- Issued in three parts: the record itself, a blank amendment form, and the index of amendments.
- A new document is issued whenever any of the sixteen entries changes — the number runs in sequence for that ship.
- Between issues, the Company or master records a change on the amendment form and notifies the Administration.
- Every document ever issued stays on board: the file is the ship's history and is never thinned out.
- Must be kept in English and produced for inspection on demand.
